Group Policy settings reverting back

I have a user that has a number of policies applied. The last policy applied
overrides a previous policy to set the screen saver as ssflwrbox.scr and does
not password protect the screen saver. These devices may work fine for a few
weeks, then suddenly revert back to the settings in the earlier policy that
set it as the text screen saver and password protects it. Then without
warning the machine may revert back to the correct settings. Does anyone
have an explanation as to why this may be happening?

Is this a roaming user that moves from PC to PC off and on?

No, it's a generic account used in a hospital setting on specific machines.
It remains logged on nearly constantly. Specific user functions are not
accessed from these machines.
