Customer Code: (combo) Customer Name: (textbox display of customer)
Customer Location: (combo based on customer code)

Supplier Name Supplier Location Distance
(supplier name)
(check box) (supplier location)
(check box) (supplier location)

I think what I am looking for is a multi-row datasheet subform but I do not
know how to start.
