getting data from combo box

My data base has one table one form one subform and a report. I want to have
a user enter data through the form and if a record in not present then enter
new data into the table via the form.

I had a similar database before but it is corrupted and I do not remember
how to modify a combo box that looks up the last name and the recalls all
information so it can be updated.

Any help would be appreciated.
