get netbios name

Anyone know of a way to get netbios name/username/IP?

I'm making a helpdesk-type DB, where the user inputs what the problem is, etc
(assuming the computer gets into windows at all).

Since user very likely doens't know the netbios name and or IP, I'd like to
include a button something like: click here if you're not sure of the Computer
Name.

UserName would be helpful, in order to get username of person filling out form
without relying on them to input it.
