Hello,

I have a questionnaire I would like to submit to my high school class reunion members.

Does anyone know of a freeware program that will allow you to produce PDF forms that can be filled in by the person you send it to?

I know you can do this with the full version of Adobe Acrobat, but I am searching for a freeware alternative.

Depends. Do you want them to be able to save the data locally? If so, no.

If you'd be content w/ their just printing out a filled in form, or posting it
to a web server which you set up, pdftex, LaTeX and the hyperref package can do
this.

http://www.tug.org should get you started.

What I was hoping for was something I could email to members of my class
that they could easily fill out and then email back to me. I would then
either print them or put the data into a database.

Neither one of the programs suggested looks like it can do this.

Does anyone else have any suggestions?

If not, I guess I will just do it as a text (.txt) document that they can
fill out or a MS Word document.

Wat about creating an html form? There are a douzain or so ready made
scripts for all kind of internet languages (vrom asp, cgi, perl and php to
javascript, visual basic script and flash. You just need to adjust them and
put in the right email adress. Mail the link to everyone you know and asktem
so fill in your questionaire. There are enough freeware webhosting providers
with php/perl/cgi possibilities. or you might even try the System
Administrator of your school. If he isn't a real ***hole and has some time
to spare (ask very politly, invest in a bottle of good whisky, ect,
ect...;-) he/she will help )

There are even online quizes/polls creators (never tested them though).

I suggest you do a google yourself, since I only encounterd them while
searching for ohter scripts. I nevr botherd to bookmark or write down the
adresses.

=============

There once was available a freeware product called:

Canon Mediator ver 5 ... a package of almost 20 Meg, which did very
much what the full Adobe Acrobat package does today.

The URL for downloading was: http://tinyurl.com/yt76j

I say WAS, because the site doesn't seem to response anymore.
If it becomes an impossibility to find, I am happy to upload it to somebody's
website if they want to offer the space. Remember, it's 20 meg in size.

Found it! Canon Mediator 5 (Adobe Acrobat clone) 20Meg size
It's registerware. Do you know if it's still possible to install - does
the Canon registration process still work?

A working download here

Its a Spanish site, translated by google.

The file I got from another site won't instal because the Canon
registration system doesn't respond.
It is possible to install (hint FAX) but Mediator does NOT output PDF

C

Better hint please
at the start of the install there is an ini file in a temp directory
that has a line controlling whether or not you get the opportunity to
register by fax. Edit, save and proceed with entering details.

Not in PDF, but in HTML, there is a software pachage that will build web
forms for you. It's on Pablo Van Der Meer's site.

I know you can do this with the full version of Adobe Acrobat, but I am

If you want to use the output from a current application, rather than learn
something new like Open Office, there is a web site
(http://www.ps2pdf.com/convert/index.htm) that will convert postscript files
to PDF for you without charge.

What you do is
- install the driver for a postscript printer
- print the output from your program to this printer but save it to a file
- go to the website
- identify the file to be converted
- the website creates a PDF version of your file that you can open and
download back to your computer.

The great advantage of this site is that it lets you convert the printed
output from _any_ application to a PDF.

Thank you to everyone that has responded to my quest.

I will look into the serveral options that you all have suggested and
hopefully will find a way to get the alumni information sent to me in a
form.

Respectfully yours,
