I want to use a guest book, but I would like re-arrange the fields on the
results page. Can anyone help?

I added some additional fields above the Comment Box. I would the results
page to appear as displayed below.
Is is possible to re-arrange the fields to have them appear across as follows?

FName: LName:
State: Email:
Phone:
(Comments)

You can put them in any order you want. Do the layout in tables, to keep
the fields lined up properly, and they should be fine.
