Something's not right in the land of Exchange?

I am currently at a company that has created several custom Outlook forms
that are used to process employee information on Exchange Servers.
Recently, forms have begun to copy themselves in folders. If this occurred
every time when the forms are forwarded to a folder, I would say: "It must
be a code issue". But it occurs 1 in 100 to 1 in 1000 times.

Is this type of behavior a known issue in the land of Exchange? Is there a
certain condition under which the Outlook client and Exchange server will
create a duplicate item?

In testing, I have been unable to duplicate this behavior.

The current Exchange / Outlook environment:

(1) Outlook 2000 Sr-1 & Sp-3 client mix

(2) Exchange 5.5 servers

(3) All forms are published without sending form definitions

(4) The submit button that forwards the form disables itself on the first
click event to prevent the sending of duplicate items.

I have searched the web and user groups and I have found 3 other people
(dating back to 2002) asking this question, but none of them have had an
answer posted. I believe I will #4. (The conspiracy deepens.)

Not a known issue that I've heard of.

Next time, BTW, remember to post your Outlook version.
