Forms Again

This what my form looks like.

Name

Jan 8th & 9th

Jan 16th

Feb 6th

Feb 12th & 13th

Mar 18th - 20th

Apr 30th & May 1st

June 29th - July 6th

In the test box they will type in Yes or No
All I get is T17, T1, T2 and so on.
I want to have the form come back when they submit to have the dates and a
yes or no next to it.
Is there anyway the form can look and come back to me the same way the form
looks on my web site? My web site is www.shankyoujrs.com
I have more forms I need to make so they can order uniforms, evaluate
coaches with the questions that are going to be on the web page.
I went to the save fields and changed them all to those dates and then hit
save fields and it came back just like it did before. T-17 and so on. I have
all the replys coming back to my e-mail address. --

Your form fields are not really connected to your form labels, you may also want to put them in a table within the form so you can control the text layout better.

But anyway...right click on each form field and rename them to anything y ou want...maybe JAN8_9, JAN16, FEB6...whatever.

As Rob mentioned previously, you need to rename the form fields. The T1, T2
etc thsat you see is FrontPage's way of naming the form fields. So if T1 is
intended to be name, then right click on the text box, select Form field
properties and change it to name. Address would be address and email would be
email etc.

As to the specific dates, you may wish to use a drop down menu or radio
buttons if what you want is yes or no.

rename the fields to whatever you want - T1, T2 etc are the default fieldnames
that Frontpage assigns.
