form to mail

everytime i click the submit button on my form it always say object not found and in the address bar it has "---webbot-self---- word, how can make my form work? help please
 
Check with your hosting provider, looks like a FrontPage
server extension error.

Check with your ISP / WPP to have the FP Server Extensions installed.
