Form Error When Submitting

I'm testing my contact form (www.adzi.net/contact) anbd whenever i click
submit i get a 404. The address it's trying to connect to is
http://adzi.net/--WEBBOT-SELF-- which is not my confirmation page...

Help Please!

1. Does the live/remote server have the FP extensions installed?
2. If yes, did you create the form on the server or locally in FP and then publish via FP's Publish
Site command to http://www.adzi.net, as the FP extensions must be installed for the form to work, as
well as the web host must have configured the FP extensions to process email from forms.

My tests indicate that the FP server extensions are not installed on the
host server. They are required in order for the form to work.

Your form properties is referencing a result page located on your C: drive.
Open your website in FP before working on a page.
