form to collate feedback from staff presentations

Hi

I need a form to enter information collated from a staff questionnaire
following a presentation. I need to gather the info and present a general
overview in a easily understandable format. From this info I would like it to
be presented in some type of chart

Hi Tessa,

This is a much too general question to be fully answered here. If you
are reasonably familiar with Access and the questionnaire has not yet
been designed, I suggest you download the AtYourSurvey sample database:

For Access 97:

For Access 2000 and newer:
http://www.rogersaccesslibrary.com/Otherdownload.asp?SampleName='At Your Survey 2000'

Otherwise, it will probably be simpler to work in Excel.
