Form Template with fillable lines

I am creating a form template that needs to be both fillable in Word
on someone's computer screen or printed out and filled in by them. I
have put in the fields where I want them and typed my text. My two
problems are with putting in lines that can be typed on and keeping
the fields from moving around when I type in them. I guess I need to
know how to put the lines in each field and then how to keep
everything from moving when it is typed on.
