Form Requiring a Password with Frontpage

Any help would be great.
Our contact us page is requiring a password and username, and I can't
figure out how to get rid of it. My HTML code is like my Spanish, I
know enough to get into trouble, but not enough to get out of
trouble. On our other website, the forms work just fine. I tried to
copy/paste the other form, but that didn't help. I have uninstalled/
reinstalled FP extensions twice, but that didn't fix it. The page is:

Make sure that the _private folder has server write permissions set for it.
That's where the form results are saved, and if permissions are not set for server write,
the user will be prompted for the user name and password.

This may sound stupid, but how do I make sure the _private folder has
server write permissions set for it.
