form properties problems

I have a feedback form and in the submit button, the form properties needs to
know where to store the results in order for visitors to submit their
feedback. When I fill out the form and press submit I get a message that says
'URL NOT FOUND"

How do I fix this?

You must set this under the form properties, not the submit button and the host must have the FP
extensions installed and configured to handle this, and you must have published (File Menu | Publish
Site) to the server via http, not by any other method.
