Form Linking to Next Form

I would like to set up forms in Frontpage that will link togther. In other words... these forms are for research in my graduate program. So i want the users to read the consent form, click submit so i have it on record they clicked it as implied consent, clicking should take them to the demographics form, then clicking submit on this page should take them to the actual survey. The confirmation page that results when the consent form is clicked has many fields on it (none of which are on the consent form) and takes you to the actual survey rather than the demographics despite the fact that I have gone into the options and clearly selected demos.html as the confirmation page.

any ideas or help is very much appreciated!!

Can not be done directly with FP, requires the use of server-side scripting,
such as ASP, PHP, etc. and maybe a database.
