Form Info Not Sending To Database

Hello,

I have created 5 seperate form pages that share 1 common database.

When a user fills one of these forms out and clicks submit a confirmation page is generated.

When I go to find the Access file with the results of this submitted form I only find the correct column titles with nothing recorded below.

How do I find the results to these submitted forms?
