form in word for outlook

is there a way to create a form with protected fields in word and saved as a
template and then create a button on my outlook template, that when I go to
compose a new mail message, I can click on a button from the toolbar and the
form opens up - ( this is a small form I use alot). Then when the person(s)
receives the message, all they have to do is fill in the information and send
back.
This will allow the user to not forget any information I am needing since
they will all be using the same form. This is something I will use
frequently.

You'd need to send the Word document as an attachment.

Depending on your Outlook version and mail server and who the recipients are, an Outlook custom form might also be an option.
