form fields and page footer

I have a word doucment with form fields in it.

At the footer of the page, we have student name and date of each page.

I know I cannot add form fields in footer, so is there a way have the
student name and date populated automatically on each page?

Apply a unique style to each form field that should be repeated in the
footer. Then insert STYLEREF fields that reference each of these styles. See

Thank you.

I know now I can use ref to do that too. But the problem is the user has to
unlock the form and view header and footer to update it.
We thought we will not let user to unlock the form so that the template is
protected, This lose the purpse of that.

You have missed the point. The reason for using a StyleRef field is that a
REF field is not practical. Go back and read the information Stefan referred
you to (beyond the first paragraph, which tells you what you *can't* do).
