Form displays blank

I have created a form that links to a table. I've setup for the form to open
from the main switchboard. When I open the form from the switchboard it
shows a blank gray box. The table is currently empty but I want the form to
show the blank fields so data can be entered. Allow Additions and Data Entry
are set to Yes. Any help on this would be greatly appreciated.
