Flagging emails in OL2003

We have OL2003 running on XP Pro and Exchange 2003 backend. I have a user
that says when he opens an email (read or unread), closes it and decides to
flag it by going into Actions - Follow up then selecting the red flag, 1 in 3
times it doesn't "flag". I know it's a minor issue but this guy is a Partner
in a Law firm so these things are important to them. Anyone got any ideas?
