Flag does not work in custom contact form

while using a custom contact form when i click on the "Follow Up" the "Flag for follow up" window comes up ... i then select what i want ... click on the "Ok" button ... the fallow up message is not desplayed on the contact form

when i view a contact on the default contact form the follow up flag works

how do i fix this?

This is a known issue with customizing the first page of the contact form.
You would have to provide your own text box control to display the flag
information.
