Fill Boxes and radio buttons

I am trying to add a box to a template where I can click and type in text in
a specific location...not just a text box. I cannot remember the toolbar in
earlier versions of Office to find that option or the option to add a check
box. Those options had their own toolbar, but I can't find anything that I
want for this in Office 2007. Help!

Go to Office button > Word Options > Popular and check the box to show the
Developer tab on the ribbon.

Now click the Developer tab. In the Controls group, the fourth icon in the
second row is "Legacy Tools". Click that and you'll find the old Forms
toolbar stuff there, except for the Lock icon (which you can add to the
Quick Access Toolbar from the Commands Not in the Ribbon category in the
Customize dialog).
