Files and folder out of sequence when open/save

Win XP Home, all critical updates applied

Suddenly, when I want to open or save a file, the folders and files
are totally out of sequence. In most cases, not always, the most
recently accessed files and folders are towards the end of the list.

How did this happen?

How can I fix it?
