File Upload Problem

After setting up a file upload form, I receive the following when I test it
on the live site:

Form Validation Error
Please correct the information you provided by following these steps, then
submit the information again:

a.. Cannot open file "PowerBuilder Change Control.doc" for writing.
The upload directory is set to allow anonymous uploads

HELP!!
