"Field on this form requires a value" Error

I have created a custom message form and published it in my personal
library. I'm able to fill out all available fields as well as put
attachments, but I'm getting an error when I try to send it. I'm getting the
message "A field on this form requires a value".
I've looked at all the fields and there are some label fields that I've
marked read-only. I tried to put a value in those fields and to leave them
with no value. When I do put a value it wipes out the label I've typed. But
either way I'm still getting the message.
Any advise?? This is the first form I've designed and would appreciate any
help.

Unfortunately, it might be a validation requirement left over from a field
you think you are no longer using. Myself, I would start over with a new
blank form, and build the design up from scratch, making sure I only use
fields that will be in the final form, and making sure that only the ones I
want to have validation have ever had it applied. I would use new field
names also.
