Favicon code

My favicon shows up in "Favorites" menus, but never in the browser bar. What
am I doing wrong? Here is the code, which I put between my last meta tag and
my script tags:

<!-- favicon -->
<link rel="icon" href="favicon.ico">
<link rel="shortcut icon" href="favicon.ico">

1. Remove <!-- favicon -->

2. Close IE and delete any shortcuts and favorites that you may have for your site.
Delete your temporary internet / cookie and history files.

3. Reopen your website.
4. Create a new favorites entry.

Thanks, Steve, I had already tried that both at home and at work (except for
the deleting <!--favicon-->, which I just added today to see what would
happen & will now undo).
As I said, the icon shows up in Favorites, but not in the browser bar or
tab. Could the problem be that our url is something of a redirect?
(www.nacc.edu goes to www. nacc.edu/mainsite)
