error publishing form

Hello.
Using outlook 2k3 and exchange 2000 sp3 8/2005 hotfilx rollup.
Created form and published it under public folder several days ago. Group
and i have been using it successfully. Today, i can no longer design/edit or
publish it. When i try to design the form located in public folder, receive
error "form load failed." When i try to edit/design and publish to another
folder, receive, "an error occured while going to this folder."
MS kb articles 817889 & 839804 were not helpful.
Any assistance would certainly be appreciated. Thank you.
