When I open Outlook 2003, I get a message "An extension failed to
initialize. Cant open file: extend.dat"

If I click OK on the message, Outlook 2003 opens OK.

My Outlook file folder shows such a file there but apparently Windows 2000
isn't finding it.

The extend.dat file is usually found in a hidden folder in Windows 2000, so
search in hidden folders. When you find the file, you can delete it --
Outlook will just create a new one.
