error message when logging into windows that has no text in

Someone I work for got this error message when they were logging into windows
that has no text in it, simply “yes†and “no†buttons. An image of it can be

.. does anyone have any plausible reasons this might show up? They clicked
“yes†but everything then continued as normal. Some other coworkers have
gotten the message several times, never having anything bad happen
afterwards, but we were just wondering what might cause it. Thanks.

Does this error occur when staring in Safe Mode? If not you can try some
clean boot troubleshooting to identify what is triggering the error.
