TOOLS | EMAIL ACCOUNTS | DIRECTORIES | "View or Change...".
Select your Outlook Book service, than change. On the next page, delete the
empty Address Books entries.
After restart Outlook they should be gone.

There is either a Tools>Accounts item (for OL 2000 IMO) or a Tools>Services
item (for OL 2000 CW). WIth the latter you should see an Outlook Address
Book service. Select it and click Properties. Select the extra folder and
click Remove. If you're using IMO, I can't help since I don't have access
to a system with that Outlook mode on it.
