Does Excel 2003 have the Add-in "Template Wizard With Data Tracki.

The Add-in Template Wizard with data tracking allowed you to enter data in a
form you had created in excel and then would automatically enter that data in
a separate database. You would then have a hard copy of a filled in form and
separate records in the database which was a different file.
