deleted switchboard form from Forms Object how do I put it back?

I am working in Access 2003
I deleted a switchboard form from the Forms Object, it is visible in the
switchboard manager, but I am unable to get the form to show up in the Forms
Object after having rebuilt it in the Switchboard manager.
my macros run ok
anyone have any suggestions.
