How do I recreate my deleted switchboard FORM?

I have the switchboard but the form somehow was deleted.
Can I recreate the form without recreating the switchboard?
 
Try this -- BACKUP DATABASE BACKUP DATABASE
Rename thable Switchboard Items temporarily.
Create Switchboard, delete the table name Switchboard Items that it
generates, rename you orginal Switchboard Items back again.
Try it.
