Custom reply form

Hi
Using Outlook 2002 on exchange server.
I have done a message form for colleagues to send to us
to report problems. What I want to is click the reply
button and have a reply form that has text and choice
buttons appear. This can then be sent to the colleagues as
a reply.
I searched here and the net but cannot find a way to do it.
Can anyone help please?
