Custom Outlook Forms

I've developed a form with some checkboxes on it. When I
send it to my self I get it, but when I start inputing the
some note in the message box and start checking some of
the checkboxes, I try to reply and I don't get that page
anymore.

What I am trying to do is send out this form to some
users, have them fillout the form and reply to me. I
should be able to see everything they've done on the form.

How can I do this?

Did you publish the form definitions with the form? What version of Outlook
are you using?
