custom listbox item w/ multiple checkboxes

Can anyone give me an idea of how to create a listbox with list items that
look like the following:

-------------------------------------------------
DatabaseUserName1
[ ] Select [ ] Update [ ] Insert [ ] Delete
-------------------------------------------------
DatabaseUserName2
[ ] Select [ ] Update [ ] Insert [ ] Delete
-------------------------------------------------
DatabaseUserName3
[ ] Select [ ] Update [ ] Insert [ ] Delete
-------------------------------------------------
etc.

Some sample starting code or reference to a url with more info would help
greatly.

Don't create one. Buy one. There are many components that do this
available. Search on SandGrid and its competitors.
 
If I had that option, I'd certainly do it. Do you have any references that I
could access to develop one on my own?
