Just make it in paint and save it as a bitmap (bmp) file. Some icon changing dialogs won't show bmp files, just rename it from whatever.bmp to whatever.ico.
Icon files are just single or multiple bmp images in a single file. Generally 2 bmp per icon - one with the picture and one with what part is transparant. When using a plain old bmp the colour of the upper left pixel determines the transparent colour. However something in XP makes it black instead of transparent - doesn't matter if you are using photos.
