Custom Forms Library

I suspect that someone has developed an Outlook form for membership tracking
- more focused on home and children - I work for a church and seek someone's
custom form for this type of application.

Is there a forms library? Or someone who has created such a form?

See http://www.slipstick.com/addins/mssampleapps.htm for a list of sample
forms from Microsoft and links to other sources. I don't recall any for
church membership.

FWIW, My church uses a Microsoft Access-based program for membership
tracking. Outlook is not a relational database, which is what you really
need for that job.
