Custom forms help

I am running exchange 2000 with OL 2000 clients. I am in the process of
trying to create some custom forms for our users. The form includes
checkboxes, radio buttons, text entry fields. When I run the form and I make
selections within the checkboxes, radio buttons, and text entry fields
everything looks fine, but when I send it, the recipient gets the form but
without any of the entries(the form looks fine just no entries). I am
looking for some advice as to how to correct this. I have created this form
from scratch as well as copying similar fields from existing forms and the
results are the same in both cases. Any help would be greatly appreciated.

It sounds like you didn't click the Edit Read Layout button and create a
read layout while you were in design mode. Message forms by default have
separate compose and read layouts.
