Cross reference broken down

I have got a number of graphs in a word 03 file (but I am using word 07) and
I have given a caption to each of them.

For example:
Figure 1.1: graph
Figure 1.2: graph
Figure 1.3: graph
Figure 1.4: graph

However, when I call out the cross-reference (alt+s+rf), the followings show
up
Figure 1.1: graph
Figure 1.1: graph
Figure 1.3: graph
Figure 1.3: graph

It is annoying me because now I can't insert a caption for figure 1.2 and
1.4. I am wondering how can I fix this?

In addition, when I create a table of figure/table, word is confused. Some
of the tables appear within the list of figures. How do I fix this?

Are you saying that the issue is with the Cross-reference dialog box or does
it occur with actual captions in the document too?

Is Track Changes on? Note that any kind of numbering won't update correctly
until you have accepted all changes.

How are you creating the table of figures (table of tables)? Based on
caption labels or based on a style?
