Creating Suveys in Access

Can anyone give me suggestions on how to start creating a
survey/questionnaire in Access. The survey will have 5
responses for each question where the respondent only
selects one answer. The end result is for me to run
reports displaying how many users answered questions 1
with 5, etc and so on. Please help.
