Creating forms

[Snaff]

Is it possible to create a form that produces other forms? i.e.
Top of all forms i need the same info, but then ticking/clicking a link to
produce another form with the same info at the top but different questions
below.
I have four forms that i have to insert the same 6 lines of info at the top,
withdiffeentinfo required below!!

You could create the four forms as separate pages in the one document and
then for the data that has to appear on each of them, see the following page
