Creating forms from Word 2003 documents

I am attempting (without success) to convert some Word 2003 documents into
forms. I have documents that I use on a regular basis but I only need to
change a few words or sentences in each document when I use them. Can
someone explain how I do this?

AutotextList-Fields might be something more for a manual quick "click
through and change" run.
