create form

I have created my own form but cannot allow others to enter info without
changing the look of the form......help me fix this please
 
Entering information into a form doesn't change the way it looks.

publish the web page - then people just fill it in and submit the data.....?

I don't know quite what you're asking.....

how can anyone change the look of the form on the web through a browser, in the
normal sense of browsing the net?
