Create a form from a scanned document

I would like to know if it is possible to create a form from a scanned Word
document? I have a Word document that we have to type information into, we
would like to scan this document and use it as a form and pull the
information from the tables and put it in the form.

You could experiment with an image in the background, and controls on
top of it.

I am fairly new at this. Can you tell me how to do this or where I can find
instructions on how to do this?

I'm afraid there are no instructions that I am aware of. I am talking
about creating a new form, putting an Image control on it, resizing it
to be as large as the form itself, and loading your Scanned Word
Document in it (you may first have to convert the doc to a suitable
format), and then putting textboxes on top of that image control.
