Count records in query

My table has many entries by date. There is a yes/no field "Did you receive
a form". I want to count how many "yes" answers were received between two
dates. I have tried a parameter query to enter my dates, and adding "count"
to the total line, yes to the criteria line, but it does not return the
correct number of "yes" checkboxes. Anyone know what I'm doing wrong?
