Converting a word document into a frontpage template

I have a document that my teachers use on a constant
bases. I was hoping that I could uploaded (word document)
to our intranet and have the teacher's fill it out and
submit. Is there anyone that could help me with this
task? Thank you.

Do all the teachers have Word?

What do you mean when you say "submit"?

Would it work for them to fill it out on the web, print it, and Fax it?

Could you use Front Page Forms instead of Word to "fill
it out and submit"?
