Control Tool Box (Check Box + IF)

Hi, I'm newible in here, I wanna to using a check box plus if function,
means "If ( "the check box tick", true, false)"

If you're referring to one drawn
from the Forms Toolbar (via View > Toolbars > Forms)
(and not from the Control Toolbox)
