Contact notes being deleted

On some of my contacts the past history notes are being deleted.
Why is this and how can I prevent this from reocuring?

Please explain some basic details of your situation:

version of Outlook
devices you are snychronising to (and does it work until you synchronise)
any add-ins you are running
