Contact Forms Basics Please

I am a new user, I have a website, but my simple
contact form is not working. Can you tell me what words
to put in the fields. The fields I have are basically.

Name
Addresss
Email
Comments

Submit Query

I am totally new at this so I do not know even the first thing to do to set
it up
can you help me please so that when someone comes to my website the contact
form will send the information to my email address. Thankyou very much
